Required Forms

Caregiver Background Check (LIC 995F; view only)

All adults who are caring for children are legally required to undergo a criminal background check. You deserve to know the process. This form is only for your record; you do not need to submit anything for this.

Identification and Emergency Contact Information (LIC 700)

Please provide the contact information for emergency situations and the individuals authorized to pick up your child from our home. This is a PDF and can be saved and emailed or printed and scanned back to Keri.

We follow safe sleep guidelines and would like to know how your baby currently sleeps. This is for informational purposes only. Sleep plans will be discussed directly with parents during the first week of care. This is a PDF and can be saved and emailed or printed and scanned back to Keri.

This agreement outlines our nursery policies, payment policies and how to end care when the time comes. This is an online form and will be submitted to Keri after you submit.

Please read before beginning: Some of these forms are provided directly through the Community Care Licensing Department and are required for enrollment. The forms must be downloaded directly from the licensing website to ensure the most current version of the form is being used. For any spaces that ask for "facility name," please use Robbins Nest Nursery.
